Well there are several things I can suggest. You could try switching to Focalin XR (extended release) or give him one or more additional doses of the regular Focalin after he gets home, on the weekends, and in the evenings. You could try combining Focalin with Strattera since Strattera lasts for 24 hours but is a nonstimulant and is not as effective in general as the stimulants. Another nonstimulant option is a category of medications known as alpha-2 agonists. These are blood pressure medications that are sometimes used for hyperactivity, impulsivity and agression. The main ones are clonidine and tenex, however there is an extended release version of tenex which is exlusively approved for ADHD known as Intuniv. You could also try different doses of the Focalin or try other stimulants such as Adderall/Adderall XR, Ritalin/Ritalin SR/Ritalin LA, Conterta, Daytrana (transdermal patch), Vyvanse, Dexedrine/Dexedrine Spansules. Also there are neuroleptic medications which function as mild tranquilizers. These are generally referred to as antipsychotics, but the newer and better ones are called atypical antipsychotics. Don't worry, they aren't only for psychosis.They are used for agression, irritability, insomnia, anxiety and appetite (which is good if your son's appetite is suppressed from the Focalin) among other things. I have used 4 different ones: Geodon, Risperdal, Abilify and Seroquel/Seroquel XR. I feel that Seroquel XR and Risperdal work the best but I only take Seroquel XR now because I got side effects from the Risperdal. I am also on Tenex but I am going to ask my psychiatrist tomorrow to switch me to Intuniv so I dont have to take Tenex twice a day. I also take Adderall XR in the morning and regular Adderall at 3pm. (I also take lexapro and lithium but those are for depression and bipolar disorder and i take klonopin for anxiety)
